Effective disease management reduces health care costs as it's used to manage the patient’s care on an ongoing basis, not just when a crisis arises.

Disease management programs

These programs are used in order to:

- identify employees who have or may be at risk for developing a chronic condition

- use evidence-based practices to manage the condition and avoid the risk of expensive treatments

- coordinate care among the several health care providers the employee may see.

Disease management programs can aslo include wellness programs which motivate employees to exercise, eat well and take other basic steps toward good health.
